Day services provides specialist palliative care support. Patients can be referred for assessment and ongoing support within one day services unit.
Day Services comprises of a multidisciplinary team. The unit is staffed by a nursing team which and consists of a Senior Lecturer Practitioner in Palliative Care, Junior Charge Nurse, Staff Nurses and Health Care Assistants. Within the unit there is also access to Medical Support, Physiotherapy, Occupational Therapy, Psychology, Art Psychotherapy, Complementary and Well-being Therapy and Chaplaincy support. The unit is also supported by a team of volunteers.
Day Services operates Monday-Friday, 9:00am - 5:00pm. . Following a referral you will be given an appointment to attend for an initial assessment. During this appointment a holistic assessment for your needs will be undertaken and from that a plan of care will be agreed with you. This will be dependent on your individual needs and you may be asked to attend as an outpatient for ongoing assessment and support by the individual services you require.
